When you have Medicare and other health insurance, such as employer coverage, one insurance will become known as the primary payer, while the other will become the secondary payer.

The primary payer will pay for your coverage first (up to its limits), then the secondary payer pays its share.

When you have multiple types of coverage such as this, make sure to tell your health care providers so they can send your bills to the correct payer.

How Do I Know Which Coverage Is Primary Or Secondary?

Which coverage is primary or secondary depends on the other health insurance you have.

For example, if you’re 65 or older and have health coverage from your current employer or your spouse’s current employer, your employer’s plan is the primary payer as long as there are 20 or more employees. If your employer has less than 20 employees, Medicare is the primary payer, and the health plan through the employer is the secondary payer.

How Do I Get Medicare If I Have Employer Coverage?

If you get your health insurance through your employer or spouse’s employer, are turning 65, and aren’t getting benefits from Social Security or the RRB at least 4 months before you turn 65, you can enroll in Medicare through your Initial Enrollment Period.

Granted, since you already have creditable coverage through your employer, it’s not exactly required for you to enroll in Medicare. You can even wait until the employer coverage ends and enroll during a Special Enrollment Period, but already enrolling in Medicare as soon as you become eligible will just grant you the opportunity to have more coverage available to you.
